BOYS PARENT MEETING AGENDA
Please read the Summer Program Agenda to make sure you are informed. If you have any question about the summer program please contact me at Coachkarps@msn.com.

PHYSICAL FORM
All Physical Forms must be completed and turned in by the first day of practice, July 5th. You CANNOT participate without it.

PARTICIPATION FORM
The Participation Form needs to be in by the first week of practice to make sure we place the order for warm-ups and T-shirts in on time.

PARENT DRIVERS
Parent Drivers should read the Driving Instructions for Volunteers, download and fill out the Permission to Use Personal Vehicle in Transporting Students and Emergency Information/Volunteers in Public Schools. Once completed contact Brian Ritter our driving coordinator, at brianritter1@verizon.net. Please schedule two day of driving. With your help we can make your sons season a success!

STUDENT ATHLETE CONTRACT
Student-Athlete Contract due first day of practice.

CLEARANCE/EMERGENCY CARD
Clearance Card is due July 5th. You CANNOT participate without it. No transportation or conditioning fees for summer.

TRIP PAPERWORK
The school requires each student to fill out paperwork prior to any trip. The General Standards of Conduct must be read and filled out by each student and parent. If a student needs to take medication a Request for Medication form must also be filled out. During the school year both the parents and teachers must release the student from class to permit participation in the trip. The Parent/Teacher Permit form requires the signature of the parent and all teaches whose classes will be missed. All necessary forms must be filled out completely and turned in to the coach prior to participation in the event.
